Meet Moses Hudson-Knapp, RN – Community Health's New Director of Population Health & Quality


Rutland – Community Health has promoted Moses Hudson-Knapp, RN, to Director of Population Health and Quality. A graduate of Castleton University's Bachelor of Arts in Psychology and Associate Degree in Nursing programs, he has served as a nurse clinical leader and clinical project coordinator for Community Health for the past seven years.

"I think I bring something a little bit different to the table that really comes from a lifetime of deep connections to this community," he said. "I have been fortunate to work with kids and adults, with people everywhere from the classroom, to the clinic, to the hospital bedside, and to blend those experiences with a professional and educational background in data analysis and quality improvement."

"With population health and quality we get to bring all of our understanding and resources to bear on improving outcomes and access at both ends of the microscope, helping to deliver the best possible care at the patient level and addressing the challenges faced by the larger populations those individuals make up," Hudson-Knapp said.

"Population Health is a very important part of what we do at Community Health. It's about being proactive and helping patients take care of themselves through prevention. I'm very excited to have Moses Hudson-Knapp take on the role of Director of Population Health & Quality for our organization," said Mike Gardner, Community Health CEO. "Moses has tremendous healthcare experience, and he knows the community well."


With over 18 years of healthcare experience, Hudson-Knapp has worked in academic, behavioral health, hospital and primary care settings including as a surgical care nurse at Rutland Regional Medical Center and a therapeutic case manager at Rutland Mental Health.

"We have some fantastic initiatives in progress, designed to address social determinants of health and both acute and chronic illness," he said, programs that focus on access, care management, homelessness, diabetes, food insecurity and patient education or support groups related to the sensitive healthcare and cultural needs of the population.

Born in Rutland and a lifelong Vermonter, Hudson-Knapp grew up in Fair Haven where his minister father and schoolteacher mother raised him in a family steeped in the traditions of service and learning. He joined Community Health Castleton in 2017 and eventually took a position working with Chief Medical Officer Dr. Bradley Berryhill. "I have spent the last several years working clinically and strategically with our staff, providers, leadership team and external partners to refine and expand processes for the delivery of care and collaborative oversight."

"I think Rutland is really fortunate to have so many skilled and compassionate people here and what's really exciting is realizing what we can accomplish when we work together as a community for our community," Hudson-Knapp said.

A lifelong learner, Hudson-Knapp also enjoys biking, kayaking, martial arts, enduro motorcycling, music and mindfulness.
