"DRIVE UP AND POKE!" RABIES CLINIC for dogs & cats!  RABIES, A DEADLY DISEASE, IS IN VT. PROTECT FAMILIES & ANIMALS BY VACCINATING & REGISTERING DOGS AT YOUR TOWN OFFICE. Dogs are required by law to have a rabies vaccination by 6 months of age & licensed.

  1.  Mail this completed form to VT Volunteer Services for Animals Humane Society (VVSA), PO Box 100, Bridgewater, VT 05034 with a check for $13 per animal. If your form doesn't arrive by Wednesday, March 24th, you'll have to pay cash at the clinic.
PREREGISTERING WILL SAVE VALUABLE TIME!  HOWEVER:
  1. You can send this form via email: skaskiw@vermontel.net.  with ALL needed information and email no later than Wednesday, March 24th. Exact cash will be collected at the clinic if the paperwork is received by email in time.  If either isn't possible, fill  
out the form OR transcribe all information onto a piece of paper and bring with you with the exact cash. This will save some time.
     3) People are welcome to show up that day and fill out the paperwork while in line. Please help spread the word!
The "DRIVE UP & POKE! RABIES CLINIC" is Saturday March 27th, 12-2:00 in the Parking Lot of the Springfield Riverside Jr High School. A single line of cars will be formed. People & animals stay in the car. Cats in carriers and dogs on a leash. MASKS ARE REQUIRED AT ALL TIMES!   Questions?  Call VVSA HS: 802-672-5302
The VT Spay Neuter Incentive Program (VSNIP) applications will be available (or send a SASE to VSNIP, PO Box 104, Bridgewater, VT 05034) to apply to help neuter your cat/dog. Funded only by a $4.00 fee when licensing dogs, VSNIP is under the VT Economic Services. If eligible, the cost to you is $27.00. ALL DOGS MUST BE LICENSED AFTER BEING VACCINATED.
