Community Health Case Managers Receive Certification
Primary Care and Pediatric Patient Services Include Case Management at Community Health
RUTLAND - Certified case managers are clinical experts who keep patients on track and are aware of the routine maintenance and self-care needed to keep an individual patient healthy. "In today's health care environment, patients/clients need an advocate on their side," said said Claudia Courcelle, RN, BSN, MSA, Community Health's Clinical Director, a certified case manager herself. "A board certified case manager is the right professional to act in an individual's best interest in a complex and fragmented health care system to provide optimum value and desirable outcomes for all involved."
Five case managers at Community Health have earned the Certified Case Manager (CCM®) credential from the Commission for Case Manager Certification.
"Case managers with this credential have the expertise, knowledge, and professional experience to provide the right services to patients across the continuum of care, including those with serious or complex medical conditions, and/or catastrophic injuries and illnesses," Courcelle said.
The CCM® credential means that these case managers are committed to uphold the highest professional and ethical standards. "To maintain certification, case managers must comply with the Code of Professional Conduct for Case Managers, which is enforced by the CCMC," Courcelle said.
